I declared that all of the information entered on this Personnel Detail / Application Form is true and correct at the date shown below and acknowledge that Oceanic Offshore Pty Ltd may require further information in regards to detail/s supplied prior to any offer of employment.
I hereby agree for all payments by Oceanic Offshore Pty Ltd("the company") for services supplied to the company to be made by electronic funds transfer and for payment of superannuation contributions made on my behalf by the company to be paid into the nominated accounts as per Section 5. of this application form - Instructions For Payment.
I also agree that if any of the above circumstances change from the date that this form is completed to any offer of employment in the future that I will advise Oceanic Offshore Pty Ltd of the nature of the said change in circumstances in a timely manner (this specifically applies to section 5. & 6. - Workers Compensation Claims & Medical Questionnaire).
